The Horse Boy tells the story Rupert Isaacson, his wife Kristin, and their autistic son Rowan. Rupert, a travel writer, and Kristen, a professor of psychology at the University of Austin, discovered that Rowan was autistic when he was two years old. They made the usual rounds of treatment options, but they often despaired of ever being able to communicate with him, or even get him potty trained.

Rowan was calmest outdoors and loved animals. One day, Rupert, a skilled horse trainer, noticed that his neighbor's lead mare dipping her head submissively toward Rowan. The neighbor agreed to allow Rowan to ride the horse whenever he wanted. Father and son rode that horse almost daily. Rupert noticed that Rowan seemed to make progress when he was with the horse, and conceived idea that even he thought was crazy. He took Kristin and Rowan to Mongolia to ride horses there and visit shamans to see if Eastern healing practices could succeed where the best Western ideas had not.

Being a published author of both travel books and articles, Rupert wrote a book proposal for The Horse Boy and received a substantial advance and a film crew. In Mongolia, the Isaacsons found themselves submitting to practices that seem very strange to the Western mind. There is no "cure" for autism, but between the sensory stimulation provided by the horses and the Mongolians' acceptance of Rowan's poor social development, Rowan made considerable progress.
